MyHealth Program
BigStep Arizona's Community Health Worker wellness program is designed to help individuals and families take an active role in improving their health. Through personalized health education, coaching, and support, our Community Health Workers help participants develop healthier habits, understand and manage chronic health conditions, navigate healthcare services, and connect with community resources
Diabetes and pre-diabetes
High blood pressure / hypertension
High cholesterol
Obesity and unhealthy weight
Heart disease risk
Poor nutrition
Physical inactivity
Asthma and other chronic health concerns
Individuals at risk for developing chronic conditions
Individuals having difficulty managing an existing health condition

Help individuals successfully access healthcare services by:
Connecting members with primary care and specialty providers
Assistance scheduling healthcare appointments
Preparing for healthcare appointments
Helping members understand healthcare instructions and resources
Assistance accessing preventive screenings
Connecting members with behavioral health services when needed
Helping members communicate health concerns to their healthcare team
Supporting follow-up after medical appointments
CHWs work with participants to:
Identify personal health goals
Develop achievable wellness action steps
Track progress
Identify barriers
Problem-solve challenges
Build confidence in managing their health
Encourage healthier behaviors
Reinforce provider recommendations
Connect participants with appropriate healthcare and community resources
Many families face challenges outside of healthcare that affect their overall well-being. MyHealth also assists members when everyday barriers make it harder to maintain their health. We assist individuals with identifying and accessing resources such as:
Food insecurity
Transportation barriers
Financial/resource barriers
Difficulty accessing healthcare
Lack of understanding of available health resources
Difficulty navigating the healthcare system
Limited access to healthy foods
Social isolation
Other social needs affecting health outcomes
